NEW STUDENT REGISTRATION

Ellis Middle School embraces the opportunity to welcome new students.  New students to Ellis Middle School will be required to bring the following documents to register.

1. 2 Proofs of Residency to Verify Student(s) Lives within the Official Ellis Middle School Zone. (lease, mortgage, or utility statements only - examples: water, electric, natural gas)

2. Immunization Record on Tennessee Form.  (please visit the local health department to get out of state immunization forms transferred) (7th graders are required to receive an updated immunization)

3.  Birth Certificate

4. Please complete the below Google form to support our efforts to maintain the most essential student information.

All other documents will be received through a records request from the previous school attended.
